Farewell IMSS, ISSSTE and other health institutes? Mexico bets on a unified system

In this, it refers to the process of centralization underway in the country with agreements between the States and the IMSS-Wellness, already signed in at least 15 entities, so that the federal government takes over the management of the operation and the attention of health services which were lost in the 1980s when this faculty was moved to the area.

The head of the Mexican Social Security Institute, Zoé Robledo, indicated that there are already 15 states with which he has signed an IMSS-Wellness agreement.

“What do we do now? to put it on one line, it’s the reverse process: centralizing that part of the medical attention that states have today (…) We are starting to work to make this model of care the same across the country as a necessary minimum of what a unit should do, especially at the second level,” he said.

The process that we are going through first goes through one element: the agreement with the state governments, there is no legislative reform that requires them to say “you no longer have competition in medical care”, and with the agreements, this part is ceded to IMSS-Wellness.

Since last year, the process of centralizing services in Nayarit has begun, to which have been added Tlaxcala and Colima, three of the most advanced states in the application of the model.

This program, which is the most important health program of the federal government after the health emergency due to the pandemic, has reinforced its budget for this year 2022 from 13,607.6 million pesos in 2021 to 23,689.9 million pesos this year.

The Chairman of the Social Security Commission in the Chamber of Deputies, Ivonne Cisneros Lujan, considered a success that the leading institution to reverse the process of centralization that has occurred in the country is the IMSS, noting that historically it has greater regulation and greater territorial presence.

“It is the most strengthened institution, historically it is the most institutional in its processes, its discipline, its prestige, despite the financial hammering, so it was necessary to find a way not only legal but operational how to do it, and the window today, because it’s inside, it’s the IMSS-Wellness, because it’s the part that has the legal possibility of serving the open population”, he said. he declares.

It is very difficult to make the right reversal by decree, it involves a whole process of reorganization and it will not be possible in the short term to reverse course and return to a vision of rights and stewardship of the Ministry of Health.
